EQUICARE CS (ECS) is a web-based software tool that can improve clinical outcomes for patients, enable care providers to operate more effectively, and facilitate successful population health management. ECS includes modules for patient engagement (through a Meaningful Use-certified patient portal), navigation, survivorship, and psychosocial distress screening.
