Welcome to Equicare’s Customer Marketing Kit. The resources here are yours to download, customize and share with patients, staff and your community to inform them about the services you provide. Items intended for print have been included as Adobe InDesign files (compatible with CS4 and later) that can be exported to PDF for professional quality printing, and as Word Documents that can be edited and printed with your desktop printer. We’ve also included several ready to use brochures that don’t require any editing or customization!

Introducing the new look of Patient portal (v4.2 and later)
If you’ve upgraded to v4.2 or later from an older version of Equicare, your patients will notice a change in the look of their icons, and several new features. Introduce the new features and interface to your patients using this brochure, which provides an overview of what’s new, and how to use it.

Patient Portal v4.7
v4.7 offers a new patient portal re-design and simple workflows. v4.7 is mobile compatible so patients can access the portal from their mobile phone, tablet or desktop devices. If you’ve upgraded to v4.7, you’ll also notice support for multiple languages. Patients can now benefit from ECS whether or not they speak English. v4.7 currently supports English and Spanish.

Patient Portal v4.9 and later
The latest version of Equicare builds on the enhancements introduced in 4.7 and includes more multi-language features and languages including: Spanish, Simplified Chinese, Portuguese, French Canadian and English (GB, AU, CA and US).

Patient Portal v4.10 (Questionnaire Kiosk)
v4.10 is the latest version of Equicare and introduces a new questionnaire kiosk, where patients can now complete their questionnaires while they wait for their appointment.
Patient Portal Brochure
This brochure is designed to provide your patients with an overview of how to login and use the portal. You may wish to provide this to patients along with their Registration details, and/or keep copies in your office or waiting room to share with patients. The brochure is available as a double-sided 8.5 x 11″ document, or a trifold brochure.

Patient Portal Poster

Patient Reminder Card
This business card sized reminder includes your unique portal URL, your organization’s contact details and space for a patient to write down their username. The reminder card can be given to a patients with their registration information, and kept in their wallet as a reminder of how to login.*TIP – Staple the Patient reminder card to the space on the back of the tri-fold brochure to ensure patients remember their username.
Press Release Templates
A press release can help you announce the introduction of either EQUICARE CS (ECS) or Active Patient Portal (APP) to your community and the media. Each template includes a suggested quote that you can attribute to someone from your organization and a description of the benefits your new programs will bring to patients. Letter of Introduction to physicians
This one page letter introduces the concept surivorship care planning to the physicians who care for your patients, and invites them to participate in the PCP Portal. PCPs engaged in your patient programs may have an increased rate of referral back to your organization.
This short presentation includes screenshots that provide a brief overview of the features included in Patient Portal. You can set this presentation to loop on TV monitors in your waiting room, or use it to introduce new patients to the portal during group meetings. Alternatively, you may wish to modify the presentation to share patient portal to members of your organization.
